(if you don't already have one, or if you need more than one).

Note: User information is necessary to be able to contact you about requests and to maintain the confidentiality of our records.

Your information will be treated in accordance with the Library Records Confidentiality Act (MCA22-1-1101). Although we collect aggregate usage statistics, these are not identified with individual users.
